Key Responsibilities
- Perform comprehensive patient assessments and ongoing monitoring
- Provide ventilator care and tracheostomy (trach) management
- Deliver evidence-based wound care
- Administer medications and manage patient care plans
- Coordinate care with an interdisciplinary healthcare team
Qualifications & Skills
- Registered Nurse (RN) with LTAC experience
- Strong ability to manage ventilators and tracheostomies
- Proficiency in high-acuity, medically fragile patient care
- Competence in wound care and medication administration
- Excellent assessment skills and effective team communication
