The RoleAs Care Manager, you are the heart of the operation — the bridge between our clients, their families, and our caregiving team. You'll oversee the quality and continuity of care across our client base while mentoring, supporting, and supervising the caregivers who make our mission possible.

What You'll Do
Conduct in-home client assessments and build personalized care plans that reflect each client's needs, routines, and preferencesSupervise, coach, and support our caregiving team, ensuring every client receives consistent, concierge-level careMatch caregivers to clients thoughtfully, with attention to skills, personality, and continuityPerform regular quality-assurance and supervisory visits in clients' homesServe as the primary point of contact for families, communicating with warmth, clarity, and responsivenessMonitor changes in client condition and adjust care plans in coordination with families and healthcare providersSupport caregiver onboarding,
