As Care Manager, you are the heart of the operation — the bridge between our clients, their families, and our caregiving team. You'll oversee the quality and continuity of care across our client base while mentoring, supporting, and supervising the caregivers who make our mission possible. Conduct in-home client assessments and build personalized care plans that reflect each client's needs, routines, and preferences Supervise, coach, and support our caregiving team, ensuring every client receives consistent, concierge-level care Match caregivers to clients thoughtfully, with attention to skills, personality, and continuity Perform regular quality-assurance and supervisory visits in clients' homes Monitor changes in client condition and adjust care plans in coordination with families and healthcare providers Support caregiver onboarding,



ongoing training, and compliance documentation Prior experience in home care, healthcare, case management, or a related field Genuine compassion for seniors and individuals needing care Comfort using care management software and standard office tools A valid driver's license, reliable transportation, and willingness to travel within the CSRA
