Responsibilities
Set up and break down program operations
Conduct intake and assessments for new participants
Develop service plan with a focus on financial stability and re-housing
Assist in following individualized housing plan with path to permanent housing
Support individuals to build skills and knowledge to secure appropriate housing
Collaborate with housing navigator and support relationships with housing providers
Assist participants in building skills to maintain housing once its secured
Provide individualized resources and referrals to community programs
Assist clients in advocating for services for which they are eligible
Provide employment development support
Develop partnerships with other community based organizations
Outcomes and data management into HMIS for participants activities and accomplishments
Follow-up with participants who have exited the program
Assist in oversight of interns during program operations




Meet for supervision bi-weekly
Other duties as assigned

Housing resources
