Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. In this role, you'll meet with patients and families who are considering hospice care—providing clinical assessments, answering questions, and helping them navigate an important decision with confidence and comfort.



You'll also build strong relationships with hospitals, referral sources, and community providers to ensure a smooth and supportive transition into hospice services. This is a great opportunity for a nurse who enjoys both clinical care and relationship-building.
You'll also assist with admissions when needed and play a key role in ensuring patients receive compassionate, high-quality end-of-life care.
