Are you a passionate Registered Nurse looking to make a meaningful difference in the lives of older Australians?

Our client is seeking an experienced Clinical Care Partner to coordinate person-centred clinical care and support services for clients receiving care through the Support at Home program.

Working closely with clients, families, care workers, and health professionals, you will help older people maintain their independence, wellbeing, and quality of life while remaining safely at home.

Key responsibilities include:
- Conducting clinical assessments and developing individualised care plans
- Coordinating clinical, allied health, and community support services




- Monitoring client health, managing clinical risks, and facilitating referrals
- Supporting clients and families through care decisions and service transitions
- Ensuring compliance with Aged Care Quality Standards and clinical governance requirements
- Managing care delivery within funding allocations and program guidelines

About you:
- Available to work 3-4 days a week
- Registered Nurse with current AHPRA registration
- Experience in aged care, community health, home care, or clinical case management
- Strong assessment, care planning, and stakeholder engagement skills
- Sound understanding of aged care standards and person-centred care principles
- Current driver's licence and relevant clearances
