About the Program
The Home-Based Care team at Cafs supports the safety, wellbeing, and development of children aged 0–18 years who are subject to statutory orders. The program includes Kinship Care, Kinship First Supports, Foster Care, and Permanent Care. Practitioners work closely with carers to ensure children are matched to safe, nurturing homes that meet their individual needs. The Family House program complements this work by supporting meaningful time with family,
which is vital for reunification and attachment.
About the Role
As a Home Based Care Case Coordinator, you will be responsible for coordinating and monitoring care placements for children and young people in foster care and kinship care arrangements.
You will work collaboratively with carers, Child Protection practitioners, families, schools, health professionals and community services to support the achievement of case plan goals and maintain placement stability.
This role requires strong relationship-building, case coordination, advocacy and problem-solving skills, together with a commitment to trauma-informed practice and child-centred care.
Key Responsibilities
- Coordinate and support foster care and kinship care placements in line with approved case plans.
- Maintain regular contact with children and young people, monitoring wellbeing and identifying emerging needs.
