SUMMARY: The Parents As Teachers (PAT) Home Visitor is an integral part of the Strong Start Partnership (SSP) Collaborative Team and as such, will manage required activities for integration and navigation including: outreach, welcome, screening, participation in multi-disciplinary review team meetings, as needed, reflective supervision and documentation/data entry. The PAT Home Visitor will work closely with families with children from Prenatal to ages 0-3, with other members of the SSP Collaborative Team and Community Partners, including CalWORKs staff.

ESSENTIALS DUTIES AND RESPONSIBILITIES:
Include the following as other duties and responsibilities that may be assigned:

Outreach:
• Together with PAT Program Coordinator/Care Coordinator,



develop and distribute promotional materials to engage CalWORKs families of children ages 0-3 to participate in home visiting program.
• Provide orientation information sessions to CWES staff to familiarize them with Home Visiting services.
• On-going individualized outreach shall include phone call/text reminders and check-ins to encourage participation and a sense of connection with the program, and to maximize participation for those parents who may feel most vulnerable or isolated.

Welcome:
• Accept appropriate CalWORKs customer referrals from CWES for participation in this project via a PLAN 103 CIV form.
• Implement F5MC Parent consents and an “enrollment”.
• Use relationship-based strategies sensitive to family language, culture, and context that “meet the family where they are” to develop an understanding of family values, strengths, current services and goals.
• Connect with appropriate information, resources
