We are seeking experienced and compassionate Registered Nurses and Enrolled Nurses to join our growing Community Aged Care team, supporting clients to remain independent, safe and well in their own homes.

Working across the community, you'll deliver holistic clinical care, health assessments, care coordination and education while collaborating with clients, families, GPs and allied health professionals to achieve the best possible outcomes.

Key Responsibilities

- Clinical and health assessments
- Care planning and care coordination
- Medication management and basic wound care
- Post-hospital and wellbeing reviews
- Falls, continence and health risk assessments
- Liaising with GPs, specialists, allied health and families
- Clinical documentation and reporting

About You

- Current AHPRA registration as an EN
- Community, aged care or home care experience preferred
- Strong clinical assessment and communication skills
- Knowledge of Home Care Packages and the aged care sector
- Current driver's licence and reliable vehicle
- Ability to work independently and manage your own schedule
- Willing to travel up to at least 45mins from home
