About the role Are you a qualified Social Worker seeking a rewarding hospital-based role with scope for autonomy and professional growth? Join our South Transfer of Care Team, covering Addenbrookes Hospital, other local surrounding hospitals and Community Rehabilitation Units.
You will work with adults aged 18+ to support safe discharges, arrange care and support, and undertake the six-week post-discharge review to ensure ongoing needs are met and outcomes are sustained.
Interviews will likely be held during the week commencing 21st September.
