This is an exciting opportunityto collaborate withhealthcolleagues in a multi-agency integrated service focused on Health and Social Care.The aim of this role is to reduce unnecessarysocialhospital admissions/to supportcarer breakdown and to promote independence, by assessing people in their own homes andassistingpeople to maximise independent living.
Our focus includes community support before hospital transfer and assistance in Accident and Emergency departments.As a Level 2 Social Worker, you will assistthemanager indelivering a holistic service, ensuring the safety of vulnerable adults, and facilitating prompt assistance for adults and their caregivers during crises such as falls or caregiver emergencies.



Your responsibilities will involve assessing needs for eligibility, devising care plans, and achieving or maintaining well-being outcomes.
Weareseekinga motivated, organised Social Worker with energy and creativity.You will need to beregisteredwithSocial Work England, along with strong interpersonal and communication skills.Your updated knowledge of relevant legislation, and an in-depth understanding of relevant adult social care legislation.
