Are you a passionate Physiotherapist who enjoys building meaningful relationships and delivering person-centred care?

We're seeking an experienced Community Care Physiotherapist to join a highly regarded community services team supporting older people and people living with disability locally in areas of Bayside and Glen Eira. About the role
As the Community Care Physiotherapist,



you'll provide comprehensive physiotherapy assessments, treatment and rehabilitation programs that enable clients to remain living safely and independently in their own homes.

You’ll also support community care staff by conducting functional capacity assessments and providing guidance on safe work practices and injury prevention.

Key responsibilities

- Conduct comprehensive physiotherapy assessments for older people and people living with disability.
- Develop personalised treatment plans and tailored home exercise programs.
- Monitor client progress and review interventions to achieve the best possible outcomes.
- Support clients to maintain independence, mobility and confidence in their own homes.
- Work collaboratively with clients, carers, families and multidisciplinary health professionals.
