This service supports patients transitioning from acute hospitals, primarily those requiring bLevel 2 and Level 3 rehabilitation /b, with a strong focus on care of older persons. /p h3Key Responsibilities /h3 ul liProvide high-quality medical care in a b25–50 bed rehabilitation unit /b /li liPerform bcomprehensive admission assessments /b and ongoing patient reviews /li liConduct bdaily ward rounds /b and collaborate with the multidisciplinary team /li liManage and optimise bmedication plans /b (including prescribing and deprescribing) /li liMonitor patient progress and respond to clinical changes /li liLiaise with bGPs, geriatric specialists,
