This service supports patients transitioning from acute hospitals, primarily those requiring Level 2 and Level 3 rehabilitation, with a strong focus on care of older persons.Key ResponsibilitiesProvide high-quality medical care in a 25–50 bed rehabilitation unitPerform comprehensive admission assessments and ongoing patient reviewsConduct daily ward rounds and collaborate with the multidisciplinary teamManage and optimise medication plans (including prescribing and deprescribing)Monitor patient progress and respond to clinical changesLiaise with GPs, geriatric specialists, nursing staff,
